Revella Health Ambetter Member Notifications

Program Description

Revella Health, Inc. (“Revella Health,” “we,” “us”) operates an SMS text messaging program for health plan members who have opted in to receive notifications about community-based preventive health screening events, mini-clinics, and related care coordination.

Through this program, you may receive messages including:

• Invitations to no-cost preventive health screening events
• RSVP and appointment confirmations
• Day-before and day-of appointment reminders, including event location and timing
• Post-visit follow-up information, visit summaries, and reminders about preventive care benefits available through your health plan (such as Ambetter My Health Pays)
• Care coordination messages related to scheduling and follow-up

No protected health information (PHI) — such as diagnoses, lab results, or member identification numbers — is transmitted via SMS.

How to Opt In

You can opt in to the Revella Health SMS program in three ways:

1. Online: Visit revellahealth.com/rsvp, register for an event, enter your mobile number, and check the SMS consent box
2. In-person: Complete a paper consent form at any Revella Health community event or mini-clinic
3. By text: text JOIN or SUBSCRIBE to (251) 351-1617

By opting in, you agree to these Terms and Conditions and confirm that you are the subscriber of the mobile number provided or that you have authority to consent on behalf of the subscriber.

Consent to receive SMS messages is not a condition of receiving any healthcare service from Revella Health.

Message Frequency

Message frequency varies. Up to 4 messages per month.

Message and Data Rates

Message and data rates may apply. Revella Health does not charge a fee for the SMS program.

How to Get Help — Reply HELP

Reply HELP to any message from us at any time, and you will receive a reply with contact information and instructions for getting support.

How to Opt Out — Reply STOP

Reply STOP to any message from us at any time to unsubscribe from the SMS program. You will receive one final confirmation message acknowledging your opt-out, after which you will not receive any additional messages.

If you opt out and later wish to rejoin, you may opt back in using any of the methods described above.

Eligibility

This SMS program is available to:

• Residents of the United States
• Individuals 18 years of age or older
• Members enrolled in a health plan that contracts with Revella Health (including Ambetter of Alabama / Celtic Insurance Company), or individuals who have registered for a Revella Health community event

For minors, opt-in must be completed by a parent or legal guardian on the minor's behalf.

Supported Carriers

Supported wireless carriers include AT&T, Verizon Wireless, T-Mobile, US Cellular, Boost Mobile, Cricket Wireless, MetroPCS, and most other US wireless carriers.

Carriers are not liable for delayed or undelivered messages. Message delivery is subject to the effective transmission from your network operator and is not guaranteed. Revella Health and the participating wireless carriers are not responsible for any delays in the receipt of, or any failure to deliver, any SMS messages, as delivery is subject to effective transmission from your network operator.

Changes to These Terms

We may modify these SMS Terms and Conditions from time to time. When we do, we will update the “Last Updated” date at the top of this page. If changes are material, we will notify you in advance (for example, by SMS or email) before the changes take effect. Your continued participation in the SMS program after the effective date of any change constitutes your acceptance of the updated terms.
